Interventions to improve healthcare workers’ hand hygiene compliance: A systematic review of systematic reviews

Objective
To synthesize the existing evidence base of systematic reviews of interventions to improve healthcare worker (HCW) hand hygiene compliance (HHC).
MethodsPRISMA guidelines were followed, and 10 information sources were searched in September 2017, with no limits to language or date of publication, and papers were screened against inclusion criteria for relevance. Data were extracted and risk of bias was assessed.
ResultsOverall, 19 systematic reviews (n=20 articles) were included. Only 1 article had a low risk of bias. Moreover, 15 systematic reviews showed positive effects of interventions on HCW HHC, whereas 3 reviews evaluating monitoring technology did not. Findings regarding whether multimodal rather than single interventions are preferable were inconclusive. Targeting social influence, attitude, self-efficacy, and intention were associated with greater effectiveness. No clear link emerged between how educational interventions were delivered and effectiveness.
ConclusionsThis is the first systematic review of systematic reviews of interventions to improve HCW HHC. The evidence is sufficient to recommend the implementation of interventions to improve HCW HHC (except for monitoring technology), but it is insufficient to make specific recommendations regarding the content or how the content should be delivered. Future research should rigorously apply behavior change theory, and recommendations should be clearly described with respect to intervention content and how it is delivered. Such recommendations should be tested for longer terms using stronger study designs with clearly defined outcomes.

Japanese knotweed is an aggressive alien species in Europe, North America, and Australia, causing a range of environmental problems. Eradication of Japanese knotweed is proving to be a difficult task, because the plant is able to propagate generatively by intra- and interspecific hybridization, and vegetatively from shoot and tiny rhizome pieces. Despite the economic consequences of Japanese knotweed on natural and built environments, its physiology is not yet fully understood; especially important are sink-source relations between old and young parts of the rhizome and growth of lateral and latent rhizome buds. Current methods of chemical control include three types of phloem-mobile herbicides, such as glyphosate, imazapyr, and synthetic auxins. These herbicides have limitations on their use, and all fail to eradicate the plant completely, for the reasons discussed in this review. Our aim is to suggest prospective approaches to enable chemical eradication: use of signals to induce controlled growth and development of quiescent rhizome buds; use of phytohormones, sugars, and light to increase allocation of phloem-mobile herbicides to the rhizome; use of xylem-mobile herbicides to exterminate the old rhizome parts; and use of different phloem-mobile herbicides at different growth stages.

This study presents a qualitative investigation into the effects of enforced occupational change on a professional musical identity. Interpretative Phenomenological Analysis (IPA) is used to explore the meaning of redundancy for six professional opera choristers. The paper highlights aspects of career disruption that are unique to singers who make their living using a biologically embedded instrument. Findings show the ‘opera singer’ identity to be a professional identity which consists of several subjective sub-identities. Adaptation to change is dependent on the salience given to these subjective identities. It is suggested that if more attention is given to these subjective sub-identities during a professional career, musicians may be better prepared for unexpected disruption to an established career pattern.

Background
There is an almost total lack of empirical research on stalking among juveniles.
AimsTo examine the characteristics, nature and impacts of stalking by juveniles.
MethodAnalysis of consecutive court applications for a restraining order against a juvenile because of stalking behaviours.
ResultsA total of 299 juvenile stalkers were identified. The majority were male (64%) and their victims predominantly female (69%). Most pursued a previously known victim (98%), favouring direct means of contact via unwanted approaches (76%) and telephone calls or text messaging (67%). Threats (75%) and physical and sexual assaults (54%) were common. The contexts for juvenile stalking involved an extension of bullying (28%), retaliation for a perceived harm (22%), a reaction to rejection (22%), sexual predation (5%) and infatuation (2%).
ConclusionsJuvenile stalking is characterised by direct, intense, overtly threatening and all too often violent forms of pursuit. The seriousness that is afforded to adult forms of stalking should similarly apply to this behaviour among juveniles given the even greater rates of disruption to the victim's life and risks of being attacked.
